Know Your Rights

As a client, a family member, substitute decision-maker, or caregiver, you can expect that every Timiskaming Home Support employee and volunteer will respect and uphold your rights, including:

To be dealt with in a respectful manner and to be free from physical, sexual, mental, emotional, verbal and financial abuse

To designate a person to be present with you during assessments, and to participate in the development, evaluation and revisions to your care plan.

To be dealt with in a manner that respects your dignity and privacy, and that promotes your autonomy and participation in decision-making.

To designate a person to participate in the development, evaluation and revision of their care plan.

3.

To be dealt with in a manner that recognizes your individuality and that is sensitive to and responds to your needs and preferences, including preferences based on ethnic, spiritual, linguistic, familial and cultural factors.

To be dealt with in a respectful manner and to be free from physical, sexual, mental, emotional, verbal and financial abuse

To receive home and community care services free from discrimination as per the Human Rights Code or the Canadian Charter of Rights and Freedoms.

To receive assistance in coordinating multiple services.

5.

A patient who is First Nations, Metis, or Inuk has the right to receive home and community care services in a culturally safe manner.

11.

To give or refuse consent to the provision of any home and community care service.

6.

 To receive clear information about our home and community care services in a format that is accessible to you.

12.

To raise concerns or recommend changes related to the services that you receive, and with policies and decisions that affect your interests, without fear of interference, coercion, discrimination or reprisal. 

7.

To participate in the assessment and reassessment of your needs, as well as the development and revision of your care plan.

13.

To be informed of the laws, rules, and policies affecting the delivery of the home and community care services, including the Patient Bill of Rights, and to be informed, in writing, of the procedures for initiating complaints about the services you are receiving.

Privacy and your Assessment - Know your Rights:

Timiskaming Home Support is governed by provincial privacy legislation, including the Personal Health Information Protection Act, 2004 (PHIPA), and the Freedom of Information and Protection of Privacy Act, 1990 (FIPPA), as well as related regulations and amendments.
 

By providing your consent, you authorize us to collect and use your health information and to share it with healthcare providers outside our organization who are involved in your care. This may include your family doctor, specialists, pharmacists, hospital staff, nurses, community support services, ambulance services, vendors supplying equipment and supplies, and other regulated health professionals who may be involved in your care now or in the future.

To facilitate and coordinate your services, we share your health information with other care providers using regional and provincial systems. Specifically, information about the services you receive from us and your assessment details are accessible to others involved in your care through:  Ontario's Health Integrated Assessment Record/Connecting Ontario Clinical Viewer.


Additionally, regional and provincial systems are utilized for making referrals, such as:

Timiskaming Home Support will collect, retain and use the personal information provided by you in the assessment form and its accompanying attachments for the following purposes:


▪ To coordinate and/or deliver the in-home support services you need
▪ To calculate fees
▪ To meet legal and regulatory requirements required by our funders
▪ To notify emergency contact in case of an emergency
▪ To support our internal educational activities
▪ To serve to communicate with other service providers.​

When it comes to assessment information, you can choose to: Request to see your own assessment at any time, make changes to your assessment, tell us if you do not want us to share your assessment.
